National Junior College Athletics Association rules can be clarified or interpreted by PSC Athletics Office.
Below are the necessary steps/forms that Prairie State College Student-Athletes need to complete in order to practice and compete in intercollegiate athletics at Prairie State College.
(Only complete these forms/steps if you have been instructed to by your coach)
Step 1: Confirm all final/official High School transcripts & additional college transcripts are on file at the college
- Transcripts may be sent to the college, mailed, or delivered in person.
- Official college transcripts must be provided for those who have attended and taken classes at any other college or university
- For those with college transcripts, we suggest that you order 3-4 official copies and have them sent to your home address. Bring an unopened copy to the athletic department and we will make a copy for eligibility purposes and return it directly to you to take to the enrollment center for entry into the college system.
Step 2: Complete the NJCAA Eligibility Affidavit FormÂ
Step 3: Complete the NJCAA Student-Athlete Registry Form - ONLY necessary for first year and transfer student-athletes. The link will be provided by the athletic department.Â
Step 4: Schedule a Physical & complete the Physical form (PDF) with all necessary signatures
- Prairie State offers 2 physical dates (end of July and middle of August)Â
- Student-athletes are highly encouraged to schedule a physical exam with their primary care provider if you cannot attend one of the physical dates at Prairie State College
- Please print out the form and have it completed prior to your doctor's appointment.Â
- The PSC physical form is the required form, we cannot accept high school or other forms.
- Your physical must be completed in the United States, international physicals will not be accepted do to variations in requirements.
- The original form needs to be turned into our Athletic Trainer, Heather Smott
Step 5: Complete the additional forms provided by our Athletic Trainer, Heather Smott

Important Considerations
- You must be a full-time student (minimum 12 credits) during the season of competition.
- You may only compete at the NJCAA level for two seasons. They must be your first two seasons of intercollegiate athletic competition in that particular sport.
- If you are a transfer student, you must have an official copy of all college transcripts and see the Director of Athletics.
